Contract and Waiver Forms

In order to accept an Undergraduate Research Student Grant, you must sign a Contract and a Waiver.

These forms must be uploaded to the Grant Application Portal within one week of your award notice. Prior to submitting your signed forms, Undergraduate Research encourages all students to thoroughly read the Contract and Waiver. Copies of the documents can be found here for review:
